Dunn Clinic will gladly work with you to establish an affordable cash plan or payment plan when or if necessary.

Health History | To prepare for your first visit, you may download and complete our Health History Form prior to your appointment to provide us with information to better serve your health care needs. If you print out and complete the paperwork available here, please arrive 15 minutes early to your appointment. If you choose to complete your paperwork at the clinic please arrive 30 minutes before your scheduled appointment time.
